All of US is an NIH project to document health data of a million people in the US.

Source Concept

A source concept is basically where the data original, so an EHR for example. Source concepts are mapped to standard concepts. For example, obstructive sleep apnea as a standard concept could be mapped to from separate source concepts of obstructive sleep apnea in ICD9 and ICD10.
