This has become a hassle in 2017 with the IRB. Here are the start of some notes related to the PHQ-9.
If the responses to the PHQ-9 indicate a suicide risk according to the American Psychiatric Association Practice Guidelines for the Assessment and Treatment of Patients with Suicidal Behaviors, our suicidality protocol will be started. Suicide risk is present with either 1) a response of 1 or more on question 9, or 2) at least 5 responses of 2 or 3 on questions 1 through 8.
